Hello everyone I'm head physiotherapist Akın. Today I'm here with my patient, and he did quadrilateral surgery.

And one of them, Femur precice 2 method. And one of them tibia LON method. After surgery today, we can say four week right? Yeah four week. Okey. How do you feel? I will explain in German. Thats my fourth week. The surgery was went very well. Of course I had pains at the beginning, also with getting up I had problem, but day by day it’s got much much better. Right now I can walk without the walker (assistan helper) I just used for 2 weeks. I only using crutches, with the crutches I can stand better straight, thats much better. The pains are realy less. How is your daily routine? Can you do by yourself? Actually at the beginning not so much but after few days it gets more better. You now why the first, I can say in ten day and after surgery in ten day, you have the swelling and it's your body reaction. That's why you cannot like before the surgery.

Your daily routine cannot do same. But right now it's okay, right? You don't have any problem. The swelling at the upper legs were at the beginning huge. They went down first. The swelling at the lower legs take few days more than the upper legs, because there are two more breaking bones. (Tibia / Fibula) Right now all the swellings is away. My daily routine is, to go by myself to the kitchen, to cooking to order something by myself, My daily activity like washing my face,brush my teeth, clean up my bed. Getting up by myself, I taking my cool packs. That I can put it on the wounds so that it heals better cools down better. Everthing is going very very well. Why you choose the quadrilateral lengthening? I wanted at the first only the upper legs, specific cm long That the combination will be much more effective and better (proportion) I decided to do both (femur and tibia) Unfortunatly at the lower leg the precice method wasn’t suitable because my bone canal was note wide enought for the precice nail.

With the precice method you have the nail inside of your bones so my bone (Tibia) canal was not wide enough for the nails. Otherwise I would do both with the precice method. At the upper legs (femur) you have realy less pain. Also when I lengthen, I not have any pains, even dont notice it. Which one is comfortable? The Precice method is much more comfortable,the lower legs (LON method) is more problematic for sleeping, if you want to turn at the right or left side. Sometimes the external fixators is touching each other but its not painful. Of course at the beginning its hurts, it is a big surgery not something small. But with the time it get better and better, day by day. The Physiotherapy sessions are very helpful and agrreable. Okay. I want to talk about the pain after surgery. How was your pain after surgery? First day.

The pain level was ten. ten of ten. The pain sensation was 10 from 10, it was painfull, but over the time it got better The first 10 days were painful but after 2 weeks/14 days is going better. I could sleep much much better. If you have pain, the nurses give you pain killers and the pains are manageable. Now I hardly have any pain, I can get up by myself, walking alone.

I wouldn’t thought but is getting better and better. I asked myself how will I get through these three months with this pain. But after 2 week the pain is realy less. The Physiotherapy session was very helpfull The muscles, nerves, tendens have to be stretch and here the Physiotherapy is a must. Everyone here are very very qualified, very good and very nice people and its going very very well. How was your nutrition before the surgery? And right now, how is your? Before I just eat junk food. and Cola,RedBull stuff like that.

A lot of sugar. Before my nutrition was not healty, only fastfood , to much sugar, after the surgery I started a healty nutrition and lost 2 kg weight. My goal is with the nutrition plan to lost weight and to eat healtier. You gettig here a lot of vitamins like calcium, magnesium, which is good for the bone healing. At the beginnig it is a little bit hard, because of the renounce of sugar, salt, which is at beginning not good for the swelling. They take care about the nutrion, sport, physiotherapy, medication,the bandages are changing regulary by the nurses. It is very hygienic and very clean. Why you want to do limb lengthening surgery? Is there a any special reason? I was always short, I was 1.67 m , in my childhood I was alway the shortest between my friends.

All my friend were taller than 1.75 m, 1.80 m , they accompainied me for years. And I dont regret the disicion. At the beginning, after the surgery , with the pain , I thought : what you did with yourself? but it was the best desicion what I ever choosed.

My goal is to become 1.82 m but if we have to stop at 1.80 m I have to accept it because my helath is more important. But at the first our goal is to reach 1.82 m , it not than 1.80 m is also enough. I am very satisfied and right now I reached 4 cm (lengthening is countinue Date: 07.01.2023) What was your height before the surgery? 1.67 cm. Okay. What's your goal? 1.82 cm. Okey that’s good. People asking to always,how is the life in the LLT rehabilitation center. How was your time in the center? My daily routine had changed. I wake up at 8:00 am and starting to lengthen the lower legs (tibia). One hour later I lengthening also the upper legs (femur). Than I have my breakfast and starting with the physiotherapy session. Than I going back to my room to take my medicine, than I have my next session and lengthening again my legs 0.25 mm at the lower legs ( tibia) and 1 mm at the upper legs ( femur) My bone healing is very good.

At the afternoon I starting my second session and going to fitnes at the gym Than I have my meal time and relaxing and wachting TV/playing Playstation and sleeping for 1-2 hours Than daily routine like brush my teeth, wash my face, hair, bodycare and taking my medicine. Than I going down to the social room (lunch room, restaurant) to have communication with other patients. We talking a lot (with other patient). Here are a lot of people from the USA, Japan, Italia, Germany Here you get to know a lot of people.

The daily schedule is filed from 8:00 am to 8:00 pm. Than we are at our rooms and enjoying the evening. Call with the family . The daily routine is getting day by day better. It's not boring,right? No Okay. How was your lengthening? Because you doing femur and tibia. Is it hard for you? Sometimes I waking up at the night beacuse of the pain, that’s normal. But it isn’t uncomfortable pain. It’s like muscle aching, like you didn’t do sport after a while and doing again. It’s feel like that, the pain is coming and going.

Some days you dont have any pain, a day after the pain is coming for half hour and going, but that’s managable pains , I can say its comfortable pains. Pain can be manage right? Yes. The body adjusts to the pain, the feel of pain is getting better day by day. We have a lot of devices in the physiotherapy room. What's your favorite machines? I getting everyday different physiotherapy massages, cold massages with a mashine (Thermorex) There also hot laser treatment (M6 Robotic Laser) There are also light elctro shock therapy sessions. Thats all af my favorites, but my abssoltely favorite is the electro shock therapy. The people asking always quadrilateral surgery is harder than a normal surgery. For example, just one femur operation. And how was your pain, for example, at the beginning? right or right now, I don’t feel it anymore (the pain) no pain, it’s my daily routine it’s like I had it my whole life. You really dont feel it. (pain) I am really happy that I did it. The pain is right now really less. Because of the nutritionist it is getting better, also physiotherapy is really helpful.

Yeah. I'm So happy for you because you can walk without anything. Yeah, without crutches, without walker. I can walk right now much much better sometimes without crutches. Of course at the beginnig it’s not easy. Because the bones has to been grow together. At the next 6-7 weeks the fixators will be removed. When the wounds are closed, we will go swimming, sauna, it will be much better. My goal for the next 3-4 months is to walk normal again.
